Els treballs han estat realitzat per la totalitat dels estudiants dels grups M-2 i M-3 de l’assignatura Botànica Farmacèutica durant els mesos d’abril i maig del curs 2013-14. Tots els treballs s’han dut a terme a través de la plataforma de GoogleDocs i han estat tutoritzats pel professor de l’assignatura i revisats i finalment co-avaluats entre els propis estudiants. L’objectiu principal de l’activitat ha estat fomentar l’aprenentatge autònom i col·laboratiu en Botànica farmacèutica

    The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Nest gasses as a potential attraction cue for biting flying insects and other ectoparasites of cavity nesting birds

    The presence of nestlings and other nest dwelling organisms in cavity nests alters the composition of gasses inside the cavity. Differential concentrations of gasses could be used by some parasites as a cue to localize their hosts. Here, we explored temporal variation in the concentration and isotopic signature of carbon dioxide (CO) and methane (CH) inside nest boxes of blue tits (Cyanistes caeruleus) during the nestling period (on days 3, 8, 13, 20, and 21 post-hatching) as well as several variables potentially affecting such variation. Finally, we checked whether differences in gas concentrations affect the abundance of different types of parasites affecting nestlings. Gas concentration and isotopic signature were significantly different between nest boxes and the forest during the nestling period. The CO concentration was higher inside nests than in the forest air, whereas CH concentration was lower. We expected to observe a positive correlation between the abundance of parasites actively seeking nests (i.e., blackflies, biting midges, and blowflies) and differences in gas concentration for those species that use these differences as a cue for host location. We observed that biting midge abundance was positively related to differences in CO between nest and forest air at day 20 of nestling age, indicating that this species can use these differences to locate hosts. We also found a positive relationship between blackfly abundance and differences in CH concentration. However, we hypothesize that the concentration of this gas inside nests may be related with bacterial activity; therefore, this relationship may be due to an effect of bacteria on blackflies and not to the effect of CH as an attraction cue for blackflies.This study was funded by the project CGL2015-67789-C2-1-P, PGC2018-097426-B-C21, and CGL2016-78318- C2-1-R (MINECO/MICINN/FEDER).     Microbial Communities and Associated Mineral Fabrics in Altamira Cave, Spain

    Evidences of microbial colonizations were observed in Altamira Cave, Spain. These consisted of distinct small coloured colonies, both on walls and ceiling, mainly located in the area near the cave entrance, which progressed until reaching the Polychromes Hall. The colonizations were characterized by a high morphological and microstructural variability and related to biomineralization processes. Two main types of CaCO3 deposits were related to the colonies: rosette- or nest-like aggregates of rhombohedral calcite crystals, and spheroid to hemispheroid CaCO3 elements. Colonies distribution seems to be controlled by microenvironmental conditions inside the cavity. The areas of the cave showing higher temperature, relative humidity, and CO2 concentration fluctuations presented a minor biomineralization capability

    Annual and transient signatures of gas exchange and transport in the Castañar de Ibor cave (Spain)

    The large microclimatic stability is a basic characteristic of the subterranean karst systems and causes a high sensitivity to changes in environmental conditions. High-accuracy monitoring of Castañar de Ibor cave (Spain) determined the temporal evolution of the aerodynamic processes and ventilation rate by tracking CO2 and 222Rn levels over a twelve-month period. This cave is characterized by a very stable microclimate, with high and relatively constant radon content (the mean value is 32200 Bq/m3, roughly, and the standard deviation is 7600 Bq/m3) and a moderate and quite stable CO2 concentration (the mean value is 3730 ppm and the standard deviation is 250 ppm). Beside the general patterns of cave microclimate throughout an annual cycle, some particular microclimatic processes are described with regard to the gas exchange between the cave and the outside atmosphere. There is a complex microclimatic functional relationship between the meteorological and cave microclimate conditions and the diffusion and flow of tracer gases from the fractures and the pore system of soil and host rock to cave atmosphere. Transient variations of tracer gas on cave air are controlled by natural barometric fluxes and anthropogenic forced ventilation due to uncontrolled opening of cave entrance. The short-term fluctuations of gas levels on cave air reveal distinct patterns during the exhalation process of theses gases from the net of fissures and pores to the cave atmosphere, depending on the isolation effect of soil and host rock

    With today’s leisure tourism, the frequency of visits to many caves makes it necessary to know about possible potentially pathogenic microorganisms in caves, determine their reservoirs, and inform the public about the consequences of such visits. Our data reveal that caves could be a potential danger to visitors because of the presence of opportunistic microorganisms, whose existence and possible development in humans is currently unknown
